肾小球是细小动脉伸入球囊后,分支成5-8个毛细血管小叶而构成。进入球囊的小动脉称入球小动脉,经各级分支最后形成盘曲的袢状毛细血管网,称毛细血管袢。各小叶的毛细血管集合汇成1根出球小动脉,从血管极离开肾小球。入球小动脉粗而直,出球小动脉细而弯曲,从而构成了明显的入球和出球小动脉间的压力差。肾小球毛细血管内的静水压较其它部位的毛细血管静水压高,有利于肾小球毛细血管的滤过功能。另一方面血液内的异常物质(免疫复合物等)也易于沉积在肾小球毛细血管壁上。 在肾小球毛细血管袢之间有少量系膜细胞和基质,它们对毛细血管袢起支撑作用,并有使毛细血管收缩、舒张的作用,可调节毛细血管的血流量,还能吞噬毛细血管基膜上的沉积物,以维持基膜的通透性。 肾小球毛细血管的结构也较其它部位的毛细血管复杂,由内皮细胞、基底膜和上皮细胞组成,称为滤过膜。 ①内皮细胞构成了肾小球毛累血管壁的第一道屏障,使血细胞及一些大分子物质受到阻拦而不被滤出。内皮细胞表面的负电荷构成了肾小球毛细血管壁电荷屏障的重要组成部分;可粘附细菌和白细胞,对基底膜的合成与修复有一定作用;内皮细胞的抗凝及抗血栓作用也很重要。 ②基底膜在内皮细胞和上皮细胞之间,由中间的致密层和两侧的电子密度较低的内疏松层及外疏松层组成。基底膜富含硫酸类肝素带负电荷,是肾小球滤过膜电荷屏障的重要组成。 ③上皮细胞贴伏于肾小球基底膜外侧,细胞呈多突状,形成多数足突,足突之间的空隙称为裂孔。上皮细胞有多种重要功能。是肾小球滤过屏障的一个组成部分,对基底膜合成与修复有重要作用。
